In order to serve all individuals, fees are established on a sliding scale based on family size and proof of income. Until proof of income is received, we bill 100% and then can adjust the fee when proof of income is provided. We accept Medicaid, Medicare and other third party insurance. All applicable co-pays must be paid.
Payment at the time of services expected
Before you arrive to receive a service, please remember to gather the following information:
Proof of address (a recent utility bill, rent receipt, or lease agreement.)
Proof of income (check stub, letter from Social Security, DFACS or Earning Statement from the Department of Labor.)
Proof of Insurance (Medicaid card, Medicare card, or private insurance card.)
